Patient Centered Medical Home
Patient Centered Medical Home (PCMH) is a program established by the National Committee of Quality Assurance which awards recognition to primary care offices who strive to put the patient’s needs first, coordinate their care and help them stay as healthy as possible. Saratoga Community Health Center is currently certified by the NCQA (National Committee for Quality Assurance) as a Level 2 Patient-Centered Medical Home. In addition, the practice is participating in a Comprehensive Primary Care initiative with the Centers for Medicare & Medicaid Services. Extended hours, by appointment:
Monday – Thursday: 7:30 a.m. to 8:00 a.m. and 4:30 p.m. to 7:00 p.m.
