Ephraim Back, MD, MPH, FAAFP

Program Director

Ephraim Back, MD, MPH, FAAFP is the founding program director for the Saratoga Hospital Family Medicine Residency program. He is a clinical professor of family and community medicine at Albany Medical College and has been a family medicine educator for over 30 years, serving as associate director of the St. Clare’s/ Ellis Hospital FMRP in Schenectady, NY, program director of the Institute for Family Health Mid-Hudson FMRP in Kingston, NY, and founder of the advanced practice clinician fellowship program at Hudson Headwaters Health Network in Glens Falls, NY. He has received multiple teaching honors including the NYS Academy of Family Physicians’ “Family Physician Educator of the Year” award twice. 

Dr. Back has provided full-spectrum family medicine care to underserved communities throughout his career, from the Indian Health Service on the Navajo Reservation to community health centers in Upstate New York.  With an interest in public health and epidemiology, he pursued an MPH and served as an officer with the Epidemic Intelligence Service of the CDC. In response to the opioid epidemic, he has been treating patients with substance use disorders, conducts buprenorphine waiver training for physicians across NY, and is board certified in addiction medicine. He has also practiced family medicine maternity care throughout his career and was the director of the St. Clare’s/Ellis FMRP maternity care program and founder of the Ellis Hospital FMRP Obstetrics fellowship.  

Dr. Back has two grown children, both family physicians with the Zuni Indian Health Service in New Mexico. He enjoys cross-country and downhill skiing in the winter and windsurfing and biking in the summer. 

Colleen Quinn, MD

Associate Program Director

Colleen Quinn, M.D., is the Associate Program Director for the Saratoga Hospital Family Medicine Residency Program, Chief of Medical Education and Wellness at Hudson Headwaters Health Network (HHHN) and clinical assistant professor at Larner College of Medicine at The University of Vermont.  

Originally from a small town in the Catskills in New York, Dr. Quinn has dedicated her career to providing family medicine in rural and underserved communities. After working 12 years in the farming community of Greenwich, NY, Dr. Quinn joined Hudson Headwaters.  Here she has focused clinically on women’s health and preventive medicine and has explored ways to ensure the quality and sustainability of primary care for both patients and staff.  This well-being approach resulted in Dr. Quinn initiating a team-based health care model in the outpatient health centers. She is also currently participating in the AAFP Leadership Physician Wellness certificate program and regularly seeks innovative ways to share her studies with students and colleagues. 

Dr. Quinn created a Longitudinal Integrated Clerkship (LIC) curriculum for third-year medical students that emphasized the importance of integrating social determinants of health and utilizing a systems approach to patient care. In 2018, she received an outstanding contribution award for this program by the Larner College of Medicine Teaching Academy, and in 2022, was nominated by University of Vermont graduates as the school-wide clinical teacher of the year.

Taking full advantage of the outdoor lifestyle in Upstate NY, Dr. Quinn spends her free time hiking the local mountains around Lake George, trail running, cross country skiing, gardening, and meditating. She enjoys these activities even more when her two teenage children join her!

Renee Rodriguez-Goodemote, MD, FAAFP

Saratoga Community Health Center Medical Director

Dr. Rodriguez-Goodemote has spent her career serving and advocating for marginalized populations in a community medicine model. That service is centered on integrated care incorporating substance use care, behavioral health, dental and primary care both in the health center and out in the community. Advocacy work focuses on health equity through social innovation and systems change work using the nationally recognized Baldrige framework. 

Dr. Rodriguez-Goodemote was recognized for her work with Bosnian and Russian refugee populations by the Mohawk Valley Resource Center for Refugees. She was also awarded the Saratoga Hospital Medical Staff Community Service award and most recently the Albany Medical College PATH (Providers Aspire Toward Health Equity) award. She was named a Saratoga Today Woman of Influence for her community work and advocacy. Dr. Rodriguez-Goodemote received the Albany Medical College Alice E. Fruehan Award for Excellence in Teaching and Clinical Precepting and this recognition was awarded again to the entire clinician staff at the Saratoga Community Health Center recognizing staff excellence in teaching integrated community medicine. 

Dr. Rodriguez-Goodemote currently leads the Saratoga County Communities of Excellence collaborative applying the nationally recognized Baldrige performance excellence framework to advance Health Equity for Saratoga County residents. This cross collaborative multi-sectorial group has developed innovative programs including Health for the Homeless, Food as Medicine Nutrition Rx and school based psycho-emotional health and wellness. Students have opportunities to learn and lead within a true community medicine model deploying systems innovation, data analysis, project design and project management of community directed programs. 

In her free time Dr. Rodriguez-Goodemote enjoys gardening, running, spending time at the lake waterskiing and swimming. The northeast winters are enjoyed by baking, reading and snowshoeing.

Kristin L. Mack, D.O., M.S

Lead physician, Hudson Headwaters Health Network, Ticonderoga and Moriah Health Centers

Kristin Mack, D.O. is lead physician at Hudson Headwaters Health Network’s rural health centers in Ticonderoga and Moriah, NY, preceptor for the CVPH Family Medicine Residency Program, NYSAFP Board Member, Medical Director for the local YMCA, and practices at the local nursing home.

During her residency, she completed a rural track and continues to offer broad-spectrum medical access to patients of all ages in traditionally under-resourced, rural areas. As a champion for treating substance use disorders, she teaches other physicians to provide Medication Assisted Treatment to those with Opiate Use Disorder. She was a grant recipient from the HHHN Innovation Fund for a program she developed using technology and community leaders to assist homebound patients’ access to medical care from their homes.

Dr. Mack previously chaired the Commission for Public Health for the NYSAFP and now serves as a Board Director where she advocates for issues concerning public and community health. In addition, she has represented Women Physicians at the National Constituency of Leaders, and she is a voting delegate annually at the NYSAFP Congress of Delegates.

Upon graduating from residency, Dr. Mack received the Philip Parilo Award, a teaching award from her junior residents, and she has continued to teach residents first in Schenectady, and now in Plattsburgh. She enjoys teaching Family Medicine with focuses on rural training, OUD, community health involvement and advocacy.

Dr. Mack lives in Ticonderoga, NY with her husband, Evan, who is an accomplished composer and professor and recently founded a non-profit, We Are Instrumental, to provide instruments to school children. Her sons, Carter and Henry enjoy music and sports and attend Ticonderoga Schools. Her family enjoys all the outdoor adventures that the North Country provides!

Timothy Reed, MD, PharmD

Director, Family Medicine Residency Inpatient Service

Timothy Reed, MD, PharmD is a staff hospitalist and director of inpatient experience for the Saratoga Hospital Family Medicine Residency Program. His initial training was completed at the Albany College of Pharmacy and Health Sciences after which he worked locally as a community pharmacist and helped coordinate medication dispensing at a local free health clinic. He completed his medical degree at SUNY Upstate Medical Center and his internal medicine residency at the University of Cincinnati Medical Center. He served as chief resident, where he focused on resident education, patient safety, and helping to coordinate the residency programs for over 120 residents.

Dr. Reed is currently working as part of a team of 36 providers with a focus on care of the hospitalized patients at Saratoga Hospital. The practice site has been undergoing a transition from a small community hospital to taking on a role of regional support for advanced cardiac, gastroenterological, and surgical care in support of smaller community hospitals. Dr. Reed is coordinating a group of 6 dedicated family medicine and internal medicine hospitalists who will be working with the family medicine residents on the Family Medicine inpatient service. He is excited for the opportunity to impact and learn from the area's newest family physicians. 

Sophia Conroy, MD

Sophia Conroy is a board-certified family physician practicing primary care at the Saratoga Community Health Center, an innovative clinic providing integrated care incorporating primary, behavioral health, addiction medicine, and dental care, and core faculty for the Saratoga Hospital Family Medicine Residency Program. She has a special interest in advancing meaningful access to healthcare and is involved in medical student and resident teaching.

Dr. Conroy completed her residency at Albany Medical Center, earning student recognition for excellence in teaching. During her residency, she served on the Public Health Commission of the New York State Academy of Family Physicians, where she focused on advancing treatment for substance use disorders within the Academy. She graduated from Albany Medical College with a Distinction in Bioethics.

Prior to becoming a physician, Dr. Conroy lived and worked in New York City, where she devoted herself fulltime to activism for Tibetan independence, and then to supporting human rights and civil liberties advocacy in the U.S. She graduated from Vassar College with a BA in Philosophy, and studied abroad in Kathmandu, Nepal, and Oxford, England. She grew up on a family farm in upstate New York.

Abigail Lubin, MD

Abigail Lubin, M.D., is a board-certified family medicine physician and core faculty member at the Saratoga Family Medicine Residency Program in Saratoga Springs, NY.

Dr. Lubin earned her undergraduate degree in biology. with a minor in music, graduating with high honors from Bucknell University in Lewisburg, Pennsylvania. She then completed medical school at Albany Medical College, earning her medical doctorate with a Distinction in Advocacy with a particular interest in providing more efficient healthcare to high utilizing populations. Following medical school, Dr. Lubin completed her residency at Albany Family Medicine, where she served as residency recruitment chair and developed a particular interest in teaching and in Women’s Health and Obstetrics.

Prior to her position in Saratoga, Dr. Lubin was a founding core faculty member at the LewisGale Family Medicine Residency Program in Salem, VA. There she was deeply involved in resident education and advising, curriculum development, recruitment, wellness, and medical student education. She also cared for patients in the newbom nursery and on Labor and Delivery and saw her own panel of patients in her clinic.

Dr. Lubin chose to specialize in family medicine because she enjoys providing whole-person, coordinated care to patients and families at all stages of life with all varieties of health challenges. She strives to create a caring environment in which to partner with patients in order to help them to achieve their best health. Similarly, she has a passion for medical education, deriving great satisfaction from teaching and supporting medical students and doctors as they advance in their careers, and striving to create competent and compassionate physicians she would feel proud to have take care of her own family.

Outside of medicine, Dr. Lubin enjoys making and listening to music, reading, crafting. playing obscure board games, and spending time with her family and friends!

Amandeep Kaur, MD

Amandeep Kaur, MD, serves as a Family Medicine Physician and core faculty for the Saratoga Hospital Family Medicine Residency Program. Beginning her clinical journey as a Physician Assistant in cardiology and later in family medicine, she gained valuable insights into providing culturally sensitive care to a diverse population. She completed her Doctor of Medicine at Ross University School of Medicine, and subsequently completed her Family Medicine Residency at Mercy Health System in Janesville, WI.

Since completing residency, Dr. Kaur has found fulfillment in practicing in a community setting. Her conviction stems from the increased autonomy in patient management, active involvement in patient care, and the opportunity for extensive hands-on training. Having worked as a family medicine physician in both Wisconsin and Virginia, these experiences led her to choose the community health center. She firmly believes that to achieve optimal success in patient health, a holistic approach is essential, addressing not only the medical condition but the individual as a whole.

Dr. Kaur currently works at the Saratoga Community Health Center, where she employs a team-based approach to patient care while taking social determinants into account. She believes that social determinants are important factors in treating patient medical conditions and are sometimes the root cause of patient medical complications. At the Community Health Center, we have many resources to help patients in this area, hence leading to improved quality of health for them. In addition to working with the family medicine residency clinic, she leads patient education classes on obesity and is involved in teaching medical students. Her clinical interests include obesity medicine, primary disease prevention, woman’s health, office procedure and chronic disease management. She is enthusiastic about contributing to the residency program and looking forward to nurturing future family medicine physicians.
