Eligibility
- All employees may enroll at any time by contacting the plan representative and completing the necessary enrollment paperwork. Part-time and full-time employees budgeted at a minimum of 36 hours for 12 hour employees and 37.5 for all other employees per pay period are eligible for direct deductions from your bi-weekly paycheck. Non-benefit eligible employees will have to pay the carrier directly.
