Patient-Centered Medical Home

Patient-Centered Medical Home is a program established by the National Committee for Quality Assurance, which awards recognition to primary care offices that strive to put patients' needs first, coordinate their care, and help them stay as healthy as possible. Saratoga Community Health Center is currently certified by NCQA as a Level 3 Patient-Centered Medical Home.
